The dreaded Doctor visit:
Ok here is my dilemma. I am a step mom and well to make a very long story short my husband and I have very limited communication with mother outside of a court room. We have been trying for 3 years to get her immunization record with no response and my husband's name barred from her doctors medical intake forms. Huge disaster. Finally, after tons of push and push we finally get some dates out of her for my step daughter to attend winter camp today. (She leaves in 7 hours) mind you, in the 3 year battle to get dates of immunizations all we got as a response has always been dont worry about it she is up to date. Well, no she's not.. at all. i now have to figure out what she still needs to get today by 6pm, when they leave, and I do not have the actual immunization card just some dates in a message. This is what I was given:

My step daughter is 12 and in Jr high (7th grade) is it just me or is she wayyy out of compliance and well good luck to me trying to get her up to date for camp in 7 HOURS?
HELP!
ya, I thought of that.. we called her Jr High and they said her recent immunization record is "missing" she has to provide that upon her return from off track now (beg. of April) since we brought it to their attention..ugh, and the hits keep on comin' ahh!
Oh see our school will not let you attend with out current records at the school and you are up to date. I had to get one shot for my little guy this year (6th grade) and I did but I forgot to give it to him to take to school the first day and well he was in class 5 min when they pulled him and made him call me. No class till they had the records. Atleast FL is good with that. Shame on the courts for allowing mom to hide the medical records - good luck green.
Um yeah, if thats the report you got she is WAY behind. I just found my son's immunization sheet (I keep a copy in work) and now while it's hard to read, because it's all doctor scrawl here's what I have just in the last 2 yrs alone.

There are about 4 others on here too Green that I just cannot make out the doctors writing, I can however make out the dates and they are all 2006-2008, so between the age of 10 and 12, she seems to be missing quite a few.
Sorry to hear about your situation.
EDIT: My son is the same age as your daughter and is also in 7th grade. And, they usually require an immunization sheet at the beginning of 6th grade.
It looks like she is missing some "series" os shots. Nothing I can do in one visit.. time to call the camp to see if they will let her attend anyway. Wish me luck on that call
Thank you Critter for that link.. The '08 shot was from us for her to attend Sumemr camp because mother wouldn't give up any info. Winter camp place requires more dates than they did though and it is to be notarized tonight so it has to be all complete or they wont do it. Time to call the attorney/case manager I guess.
*update: She's on her way to camp! They understood our situation and said no problem...... Whew!
